We are accepting applications for kindergarten. To enter Kindergarten, children must be five (5) years old on or before September 1.
To enter first grade, children must be six (6) years old on or before September 1.
1) Birth Certificate
2) Proof of immunization of Form 680, which can be obtained at: Department of Health, 832 W. Central Boulevard, Orlando, FL 32801; (407)836-2600; or from your doctor's office.
3) Proof of physical examination within the last year. If documentation cannot be provided, then a physical examination must be scheduled within 30 days.
4) Verification of your residency in Orange County with one of the following:

| B. Lease/Rental Agreement |
| C. Notarized Statement |
| D. Declaration of Domicile (for new residents of Florida) |
